Please read these terms carefully. They set out the basis upon which you will be provided with treatment by Wessex Cardiology LLP.
We are a limited liability partnership with registered number OC308834. In these terms, references to “Us” or “We” means Wessex Cardiology LLP.
Basis of Contract
By signing the registration form to which these terms are attached, or by attending an appointment detailed in an appointment letter to which these terms are attached, you agree to be bound by these terms.
These terms, your appointment letter and the registration form to which these terms are attached form a contract between you and Us.
Price and Payment for Care
You agree to pay for the cardiology treatments and clinical care we provide to you, referred to in these terms as the “Services”.
If you are self-funding, the price of any out-patient treatments will be sent to you with your appointment letter, and the price for any in-patient treatments will be notified to you at the time the treatment is recommended.
If you are unsure about the price of any of the Services you are due to receive from Us, please ask.
Some aspects of your care, such as X-rays and hospital stays, may be provided not by Us but by Spire Hospital Southampton. Payment for services or care provided by Spire Hospital Southampton rather than Us must be settled separately with the hospital.
Where you have private medical insurance, whilst you will remain responsible for the payment for our Services, We will, where possible, arrange for the insurance claim for the Services to be processed directly with your insurer, provided you have given Us and your insurer all the information We and your insurer need to do so.
In many cases, these arrangements will be made on Our behalf by Spire Hospital Southampton.
In some cases your insurer may not pay for certain parts of the Services you receive, and you will be required to pay for that part of the Services directly to Us.
If We invoice you for the Services or an element of them, you agree to pay Us the amount invoiced within the time limits set out in the invoice. If no time is specified, you must pay within 30 days.
If you do not think that We have invoiced you correctly, please let Us know as soon as possible so We can deal with any mistake or misunderstanding.
Cancellations
If you decide not to go ahead with your treatment, you will need to pay for the Services that you have received up until the point of cancellation.
We reserve the right to charge a cancellation fee for any appointments cancelled within 24 hours of a scheduled appointment. This will include any costs We have incurred and, where We are unable to fill the missed appointment, the fee may be the full cost of the appointment.
If you have already paid for proposed Services, We will refund your payment, less any amount that you owe to Us.
Data Protection
We collect and process the information that you provide to Us on your registration form and during the course of your consultations and treatments, including health-related information.
For the purposes of the Data Protection Act 1998, We will be the data controller in relation to such personal data.
We will use the data We collect to diagnose any illness or condition you may have, to decide upon the most appropriate treatment, to monitor your condition, and to keep in contact with you.
We may disclose some of your personal or health-related details to Spire Hospital Southampton or other health professionals to the extent necessary in connection with your treatment.
We will not store or use your personal details for marketing purposes.
By signing your registration form and agreeing to these terms, you agree that We may record the personal details We collect and use it for the purposes outlined above.

Miscellaneous
While We will do Our best to ensure a satisfactory outcome, no clinical procedure is entirely risk-free and the results of any particular treatment cannot be guaranteed with complete certainty.
Where you sign a registration form as a parent or guardian on behalf of a child under the age of 18 who is under your care, you agree that you will be bound by these terms, even if the child is not.
We may transfer and assign your contract with Us to any person who acquires all or substantially all of the assets of Wessex Cardiology LLP.
